Peritoneal Dialysis is a low cost alternative to hemodialysis. Peritoneal Dialysis carries greater risk of complications and its use is declining rapidly.Patient education is important before subjecting the patient to peritoneal dialysis.
Peritoneal Dialysis Procedure:
A catheter is inserted surgically with one end in the abdominal cavity and the other end protruding from the surface. The fluid is infused into the abdominal cavity via the catheter in position left for a particular interval of time. The waste materials diffuse from the blood into the fluid. The peritoneum acts as a dialysis membrane.
Peritoneal Dialysis may be performed by any one of the following methods.
1.CAPD – Continuous Ambulatory Peritoneal Dialysis
2.APD – Automated Peritoneal Dialysis
In APD, fluid removal is carried out once in a day (preferably at night) while in CAPD, removal is done several times during day,
Advantages and Disadvantages:
Peritoneal Dialysis is cheap and requires lesser expertise.
As with any invasive procedure, peritoneal dialysis is to be carried out under sterile precautions. Hypertension and Hypotension during or after procedure are due to lapses in monitoring which can be effectively managed by timely intervention.
Peritonitis and Peritoneal Sclerosis occurring in long standing PD patients is a cause for worry. Higher incidence of peritonitis offsets the low cost advantage.
