Bleomycin toxicity is well documented in medical literature. Bleomycin concentrates preferentially in the lung and is inactivated by hydrolase enzyme. Relative deficiency of hydrolase enzyme in lung tissue predisposes to bleomycin toxicity.
Bleomycin toxicity in lung is characterised by,
1.Early endothelial cell damage
2.Destruction of type 1 pneumocytes.
3.Migration of type 2 pneumocytes into alveolar spaces inducing interstitial changes.
Histopathological appearance of lung in Bleomycin toxicity
Endothelial cell edema and blebbing
Necrosis of type 1 pneumocytes
Hyperplasia and metaplasia of type 2 pneumocytes
Inflamatory infiltrate with PMN
Proliferation of fibrobalst and fibrosis.
