Most people encounter shoulder pain during their lifetime. Shoulder pain may be attributed to sports activities, routine work, stress, injuries etc.
Shoulder pain is usually due to inflamation of tendon or muscles in the shoulder girdle or nerve damage, termed radiculopathy(more sp. intra radiculopathy)
The nature, extent of pain, variation with active and passive movements determines the cause. So it is essential to obtain a thorough history, perform a complete examination correlate with his work, routine activities and give consideration to his age.
Posterior shoulder pain and a normal shoulder examination with worsening of pain towards night are clues to diagnosis of cervical radiculopathy.
Subdeltoid bursitis, common in elderly, causes exquisite local tenderness. Rotator cuff tendinitis associated with pain on active but not passive movements. In addition if weakness is encountered it is suggestive of rotator cuff tear.
